Dr. Katz' office accepts health insurance policies issued/underwritten by the following
insurers:
- Medicare

Blue Cross / Blue Shield insurers regardless of
the specific state, as long as your BCBS company participates in BlueCard. This includes dozens
of companies and plans, such as Independence Blue Cross (PA), Horizon (NJ), and Highmark (PA).
Both HMO (for example, Keystone Health Plan East) and PPO (for example, Personal Choice) are
accepted.

- Operating Engineers Local 825 Fund
- Motor Vehicle Insurance Claims
Please note that in some cases primary physician's referral, preauthorization, or
